The Value of T2-T2 FLAIR Mismatch Sign in Predicting IDH1 Mutation Status in Gliomas

The Value of T2-T2 FLAIR Mismatch Sign in Predicting IDH1 Mutation Status in Gliomas

Abstract

Objective To investigate the value of T2-T2 FLAIR mismatch sign in predicting IDH1 mutation in gliomas. Methods Patients with glioma confirmed by pathology and IDH1 gene subtype were collected retrospectively.All patients underwent routine brain MR scan before operation.The signal characteristics of lesions on T2WI and T2 FLAIR images were observed.The inconsistency of signal uniformity between T2 and T2 FLAIR was defined as T2-T2 FLAIR mismatch sign.Gliomas were divided into IDH1 mutant type and IDH1 wild type groups.The differences of age,sex and WHO grade of gliomas between the two groups were compared.The specificity,sensitivity,positive predictive value and negative predictive value of T2-T2 FLAIR mismatch sign in predicting IDH1 mutation status in gliomas were calculated. Results A total of 56 cases of gliomas were collected,including 28 cases of IDH1 mutant type gliomas and 28 cases of IDH1 wild type gliomas.There was no significant gender difference between the two groups (P= 0.27),and there was no significant difference in age (P= 0.11).Among IDH1 mutant type gliomas,WHO 2/3 grade gliomas accounted for 96.42%(27/28),while IDH1 wild type accounted for 14.29%(4/28),and the difference was statistically significant (P< 0.01).T2-T2 FLAIR mismatch sign means that the tumors had almost uniform hyperintense signal on T2WI,but on T2 FLAIR,the central region of the tumor showed relatively hypointense signal,with thin high signal edge.The probability of T2-T2 FLAIR mismatch in gliomas was 8.93% (5/56).The sensitivity of T2-T2 FLAIR mismatch sign in predicting IDH1 mutation status in gliomas was 17.86% (5/28),the specificity was 100% (28/28),the positive predictive value was 100% (5/5),and the negative predictive value was 54.90% (28/51). Conclusion T2-T2 FLAIR mismatch sign is a highly specific imaging biomarker for predicting IDH1 mutation status in gliomas,but its sensitivity value is low.
